Eligibility Requirements
To be eligible:
- An individual must be in a department of family medicine that is a member of ADFM. They may serve in any role (faculty or staff).
- The individuals must have demonstrated significant and sustained scientific and professional accomplishments, volunteer leadership and academic service in family medicine. They would typically have earned their terminal degree at least 10 years prior to nomination.
- Nominees will have demonstrated significant contribution to the academic aspects of the specialty in at least two or more of these areas of excellence:
- Scholarship
- Education
- Leadership
- Advocacy
Typical contributions may include:

Publication in the peer-reviewed medical literature, at least some of which is in the family medicine literature

Serving as a leader locally and in one or more state/national/international family medicine organizations, especially the CAFM organizations

Generation of educational strategies, tools, curricula that are utilized widely within the specialty

Generation of clinical care innovations that are utilized widely within the specialty
A Complete Nominations Package Includes:

Primary nomination from a leader in the candidate’s department (e.g. chair or vice chair or administrator). The primary nomination form includes requests for specific descriptions of how the candidate meets or exceeds the eligibility criteria for the award and in which specific areas (e.g. scholarship, education, academic leadership, advocacy). Template available for review.

Letter of recommendation from a secondary nominator with specific description of how the candidate meets or exceeds the eligibility criteria for the award and in which specific areas (e.g. scholarship, education, academic leadership, advocacy), as well as the nominator’s title/role(s) and how they know the nominee. Body of the letter may be 500 words max.

In addition, the candidate’s CV must be submitted as further demonstration of their accomplishments.

Nomination fee of $125 (paid by the nominating department to help cover the cost of administering the program)
The Nomination Form is available for review and preparation in a Word document here. Nomination forms are only complete once the nomination fee has been paid and the form is submitted online.
To nominate a candidate, you will first pay the nomination fee (you will have to log into your ADFM account) and in your confirmation email, you will receive the link to the nomination application.

2026 Timeline
- March - early May: Call for nominations open
- May - end of June: Applications compiled and reviewed by committee
- July - August: Recipients notified
- February 2027: Recipients honored at ADFM Annual Conference (attendance is welcome but not required)
